Health Board / Report examinations are performed during working hours on non-Thursday days of the week.
Reports;
Power of attorney is required from persons over 65 years of age for title deeds and weddings. Since the power of attorney's reports are valid only on the date of submission of the report to the notary public, it is necessary to apply in the morning on the same day and apply to the notary after the completion of the transaction.
The power of attorney reports issued upon the request of the notary public is valid only on the date of the report. therefore, it should be applied in the morning on the same day.
1-The person applies to the secretariat and receives a barcode.
2-He/she applies to the Health Committee Polyclinic with 2 photos and ID card.
If it is determined that the person examined by doctors is qualified to receive reports, the report is prepared by the Health Committee.
If the mental state is not found to be sufficient, a report will be prepared to inform the patient that it is appropriate to appoint a guardian.
Disability retirement and social assurance of nearby;
The person applies to the Social Security Institution (sgk) with a petition explaining his request.
1-Patient applies to the Board of Health with the official letter he received from the Provincial Directorate of Insurance.
2- The patient is examined by the doctors at the Health Board Policlinic. Appointments are made for the psychometric tests deemed necessary.
3- After completing the psychometric tests, the patient makes an appointment for the day of the committee after submitting the test result to the doctor.
4- The report prepared as a result of the re-evaluation of the patient on the day of the committee is sent to the institution sending the patient with an official letter.
Appointing a guardian;
The relatives of the patients who request to be appointed as guardians apply to the court with a petition.
1-With the official documents received from the court, the Health Council is applied.
2- The patient is examined by the doctors at the Health Board Policlinic. Appointments are made for the psychometric tests deemed necessary.
3- After completing the psychometric tests, the patient makes an appointment for the day of the committee after submitting the test result to the doctor.
4- The report prepared as a result of the re-evaluation of the patient on the day of the committee is sent to the institution sending the patient with an official letter.
REPORTS FOR INSTITUTIONS RELATED TO MINISTRY OF HEALTH;
The person applies to our hospital with a petition stating his profession and place of appointment.
1-He pays the examination fee to the cashier's office
2- If it is found appropriate for the person to be examined at the Health Committee Polyclinic, a single expert report is issued on the same day.
